• Ketamine therapy for treatment-resistant depression, anxiety, PTSD, and chronic pain
• Spravato (esketamine) nasal spray treatment
• IV therapy including NAD+, vitamin infusions, and wellness treatments
• Comprehensive mental health and pain management services

3. Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete medical history and current health information
• Follow all pre-treatment and post-treatment instructions
• Attend all scheduled appointments or provide 24-hour notice for cancellations
• Arrange for transportation after ketamine or Spravato treatments
• Report any adverse reactions or side effects immediately
• Pay all fees and charges according to our payment policies

4. Treatment Consent and Risks

5. Payment and Insurance

Payment is due at the time of service unless other arrangements have been made. We accept most PPO insurance plans and will work with your insurance provider to maximize your benefits. Patients are responsible for any deductibles, co-payments, or non-covered services.

6. Cancellation and Refund Policy

Appointments must be cancelled at least 24 hours in advance to avoid cancellation fees. Treatment packages may be refunded according to our refund policy, which will be provided at the time of purchase.
